QUESTION 1:
There are mainly five principles of person-centered care. The first one is valuing people. This includes giving proper respect and dignity to the individuality of a person. The support-staff should be aware of the perspective of the individual. They should consider the value system of the patient, personal choices and also their beliefs. The second one is to offer a sense of autonomy to the patient. The third principle is to give importance and value to their life experiences. The fourth one is that the caregiver should be able to understand the relationships of the individual and offer service accordingly. The final one is to offer the best environment to promote healing and wellness.
B: Explain how person-centered care can be achieved, especially when caring for a client who has recently had a stroke and has lost the ability to walk.(50-100 words)
Person-centered care will help to improve the health status of a person suffering from moving disability after a stroke. The strategy will be to facilitate patient participation in all the support activities. The nursing strategy needs to be focused on giving autonomy to the patient and probing him/her to make small initiatives towards progress. Computer-assisted devices and gaming can be used as additional tools. Offering dignity to the patient and respecting their personal choices will also help to develop confidence in them. C: Identify the risk factors associated with person-centered care. (50-100 words)
Person-centered care is not devoid of risk factors. The entire system will not work if the support-staff are not emotionally and culturally inclined to the person’s systems. Individual choices also become a determining factor to offer the best healthcare support. The role of policies and laws made at the state and national level and the approach of organizational heads can also adversely affect the system. (health.org.uk)
QUESTION 2:
Complete the table below by first filling in the sections about how a person may be affected by each of the different illnesses or conditions.
Next, identify how a support worker could encourage and assist the older person to engage actively in all living activities and to seek assistance when needed.
Two points in each box are sufficient.
QUESTION 3:
Aged care no longer focuses on the weaknesses but on the strengths of what a client can do and the choices they make. Many clients, as they age, give up on life and expect care workers to do everything for them. This is often due to the recognition of their weaknesses, however, modern methods, with an emphasis on a client’s strengths, promote individualism and empowerment. This latest industry approach is person-centered care.
Below is a list of some current underpinning approaches in the aged care sector. Give an explanation/definition of each, and an example of how you can empower the client for each. Write just one sentence in each section of the table.
