What behaviors would indicate that the client is sustaining at a healthy baseline?
It is important to establish the baseline of health for patients to help understand their status at the beginning of the treatment plan (Freedland et al., 2015). In the case of Eliza, a healthy baseline would be determined by psychological stability. It would mean that she’s not suffering the mental issues that have engulfed her over a period. The absence of situational anxiety would signal a healthy baseline. Having experienced such kinds of stress, any situation that guarantees mental stability and standard behavioral manifests would indicate that she is a healthy baseline. However, she is overwhelmed by stress and depression at the baseline.
How would you determine if Eliza met her treatment goals?
After suffering situational depression and stress, Eliza looked for treatment to help regain her health status and normalcy. Her initial goal was to alleviate stress and anxiety. Having been put on a one-on-one therapy, it was still apparent that it lacked effectiveness. Alleviation of depression and stress would indicate that Eliza met her treatment goals. However, she had to be put on other therapies to help regain normal condition.
What factors would determine if the treatment needed to be reevaluated, extended, or possibly referred to another clinician or setting?
Treatment should be reevaluated if it does not achieve the set treatment goals. It is clear that the recommended exercises are critical in alleviating depression and anxiety. The effectiveness of the treatment plan will dictate whether the client sticks to the plan or is referred to another place for treatment. However, the client was given a prescription that takes into account the effectiveness and time. Exercises are known to be effective in dealing with anxiety. Response time is also a factor in determining if the treatment plan is working as expected (Freedland et al., 2015). If the response time is longer than expected, the client can be referred to another clinician or the treatment plan reevaluated.
Based on your assessment of current symptomology, does your client, Eliza, need wraparound services, outpatient references, and step-down services?
Emotional stability is essential to everybody. Since Eliza is suffering from situational anxiety, she should be put on outpatient reference to help maintain her mental status. Depression arising from situations makes it necessary to put someone on a long term treatment plan that focuses on assessing the situation and controlling the stressors at the time of occurrence (Freedland et al., 2015). Primary care should always concentrate on ensuring sustainable emotional care. As a result, the client will be able to be monitored through the treatment plan to ensure that she goes through every plan to achieve the wellness goals. Further, the client needs outpatient references to make sure that she is getting appropriate treatment for the condition at the right time. It will enable her to reach or achieve the treatment goals quickly. Therefore, it will be prudent if the client was put on outpatient reference to help maintain psychological wellness. It will also help the client to cope with the treatment plan and ensure that she meets each and every prescribed therapy.

It is clear that the client acquired good awareness regarding her psychological conditions. Treatment goals are often aimed at creating awareness about a condition so that the client clearly understands the need for the treatment plan. The first crucial accomplishment was the awareness that she got at the baseline. The client also aimed at achieving psychological independence. The cognitive behavioral therapy was essential in addressing the client’s persistent situational anxiety. Due to the effective nature of cognitive behavioral therapy, the client indeed met the treatment goals set in the plan.
The success of the treatment plan depends on the commitment of the client to the plan. Also, it depends on the motivation that the client has at the commencement of the treatment plan (Sharf, 2015). Owing to the improvement of the client during the treatment, it is evident that the treatment plan was successful. Cognitive Behavioral Therapy mainly contributed to the success of the plan. As demonstrated on many occasions, CBT plays a significant role in the self-assessment to help one understand their ability to endure the situation (Corey, 2015). Also, CBT helps in motivating the client in sustain the treatment plan. It has also been proven that CBT possesses feasible ability in the treatment and management of depression and stress disorders. Therefore, it is clear that the use of cognitive behavioral therapy played a vital role in enhancing the treatment plan. It proved to be a success factor.
The final session is the stage at which termination is conducted. During termination, the progress of the client should be reviewed to ensure that they are ready to make a new beginning for themselves. It is important to discuss with the client their readiness to end the psychotherapy (Corey, 2015). The date will be set to ensure that the client is prepared psychologically. It is also essential to give clients the chance to speak about their experience as well as the skills they have attained throughout the sessions (Sharf, 2015). Documentation will be done during the session and the discussion with the client.
