Mission statement for the Golden Age Hospital and Community Clinic
The Golden Age Health Care (GAH) is located in Greenwood, USA since 2011 (Home Health Care Mission Statements, 2016). It provides premier health services that are home based through its specialized and paraprofessional team. Moreover, this health care offers a wide range of facilities such as expert nursing, physical and occupational therapies, home health assistance, evaluation of home safety, management of medication, neurological conditions, pre-operative as well as post-operative care. Therefore, the mission of the GAH is to deliver both professional and paraprofessional services to its customers in their respective homes. This is to help the health care in achieving the highest level of potentiality in its daily operations to self-care activities. Consequently, the hospital is committed to providing quality and several disciplinary concerns through the experts who are capable of recognizing the need for comprehensive evaluations of the wants as the point of view of the client and the professionals.
The mission statement of the Community Clinic (CC) is to provide health cares to poor people especially those that do not have insurance covers on health in the community (Community Care Clinic - Mission Statement, 2016). According to the above mission, the CC can promote health and the well-being of the community through prevention of diseases, continuous care, and active follow-up.
Simple Organizational Structure for the MVH, CHOC, GAH, and CC
Different departments in these hospitals share many services such as clinical, diagnosis, laboratory, pharmacy as well as social work. CHOC is operating a 279-bed space and serves even the surrounding communities. However, the recommended bed space for the health care is 582 as experienced in the Mission Hospital.
Organization relationships within the structure blocks
According to the organization structure above, it is clear that specialization in different departments of these Hospitals increases efficiency. However, the division of labor can also create some challenges, particularly when figuring out on how to coordinate different jobs and the individuals who can perform such tasks. The blocks in each organization structure relate to one another in the sense that it defines how every duty is assigned and grouped with formal reporting relations. The organizational chart of these hospitals effectively coordinates their operations, and this has increased the likelihood of success. Moreover, the structure is correctly matched with the needs of the hospitals; hence, there is prosperity.
The chart also indicates that there are a vertical and horizontal linkages. The vertical relations holds the supervisors and the subordinates together. It further shows that there are some lines of responsibility. For instance, the CEO can delegate duties to the line managers such as the human resource manager, and director of quality assurance for the purpose of overseeing their operations, evaluating their performance and provide guidance towards the improvement of the health facilities. According to the structure, every person except that individual at the top of the organizational chart serves as a subordinate to another person. For instance, the Board of Directors supervises the CEO, who in turn monitors a set of managers or departmental heads.
Concerning the horizontal linkage, some relationships exist between the equals in the hospitals (Edwards, 2016). In the above chart, the director of quality risk management and the Human resource manager are in the same line; hence, they are considered to be equal. Therefore, such relations may occur during the operation of the organizations in the form of committees, and teams especially when the major decisions of the hospitals are considered first for their cross-departmental influences. The horizontal relationship is also essential when considering close coordination across different departments of the HealthCare.
Services at Community Clinic and GAH
The health of the children is the main concern of the Community Clinic. Therefore, most of the patients in CC are below 18 years of age. Moreover, the clinic takes the patient-centered method to health care that is complete, coordinated, accessible, and focused on safety and quality services. Therefore, the Community Clinic is committed to the creation of a caring experience while ensuring that the children receive everything that they need for healthy living. The CC offer the following services: care for asthma, circumcision for the newborn children, immunization, and reproductive health among the teenagers, dental services, and physical education. Additionally, the pediatric department offers services to the newborn children.
The Golden Age Hospital provide the following services: care for the dental problems where the patient will receive about 25% discount on the usual and customary out of pocket services in the dental department (Golden Healthcare Group, 2016). Care for the eye issues, financial counseling, and transportation. Regarding the availability of transport, it has been arranged on a 24-hour basis and in advance at no cost. However, the availability is based on first come first serve.
Integration of services of doctors in geriatric treatment
The workforce for the geriatric health and use of the substance is defined closely to offer a set of services such as assessing, diagnosing, managing, and treating the aged people that are at high risk for the MH/SU conditions. The team from this department have to change the overlapping duties because several older people may view different types of health providers to tackle their complex range of physical and mental illness issues. However, the modern education and training for the providers of health care do not consider the knowledge and the necessary competencies to offer adequate care for the older individuals that require the care for geriatric. Therefore, the specialists for the MH/SU should be highly trained to handle complex cases, but they are low supplies. The direct care employees, the providers of peer support, and the consumers and their family members should play important roles in the workforce of geriatric. During integration, the groups are less trained, and it becomes difficult to analyze. Similarly, the problems that are facing the geriatric are many and technical. Therefore, several stakeholders have started efforts of building and strengthening the workforce with different levels of success.
In conclusion, any health care should be provided with a compassionate mission. The mission statement should concentrate on the values that can allow the hospitals to grow, flourish, and succeed. Whether a particular hospital has specialized in attending the elderly patients, rehabilitation, recuperating care, and attention for the children with special needs, both patients and the health provider should be nurtured. Consequently, a mission that is capable of communicating some personalized services particularly from the diet and nutrition to self-hygiene, good schedules for medication, proper supervision for the patients, and the handling of both Medicare and Medicaid benefits will make any health care provider to prosper.
