Program Planning and evaluation for community health
The presented health care plan is determined over the assessment and management of childhood obesity. The evaluation over the agenda has been conducted for addressing childhood over the spectrum of the area of New York State and NYC. The obesity rate of children age (2-14) of the state is 15 / 41 amongst the other states of the country. In the spectrum of the children age (10-17) the state rank 25 / 41 displaying the rate of 14.5%. The reading of the ratio is 10.6% at the level of high school student with state ranking 36 / 43 amongst other states. The implication of childhood obesity is likely to develop medical complications at the populace that include diabetes, hypertension, heart diseases, arthritis and obesity related cancers. The presented programme intends to address childhood obesity at community level.

The mission of the programme is to promote healthy habits and active lifestyle at community level and create awareness of the consequences of childhood obesity.

- Goals
The goal of the plan is to facilitate the community with awareness programmes related to childhood obesity and facilitate the community with intervention. The goal is to develop options to monitor child obesity with particular reduction approaches implied at community level and institution level.
- Objectives:
The objective of the plan is to create awareness and acceptance of the modifications in lieu to healthy life style management. The action plan is supported with reasoning of health implications obstructed under childhood obesity.
- Intervention:
The plan is to coach a healthy living style and facilitate the environment with interventions. The intervention designed under the plan is the education programme. Parents are proposed to call after school for the demonstration over child obesity and the long term influence of the same on the life style of children. The supporting material will be provided to guide the community towards healthy living choices. The importance of active environment will be demonstrated with the invitation of free of charge health activity set up. Three schools will be covered with a combined activity setup. The purpose is to elucidate the importance of active life style and coach the community for healthy life style. Also the health educator will be hired to serve the community with information regarding the public policy implications and the modifiable approaches that could combat childhood obesity.
