Dr. Wheelis' case is currently applying an affiliate model to manage The Brattle Center (TBC) which deals with patients who have borderline personality disorder. The affiliate model is based on performance and therefore Dr. Wheelis rewards TBC staff based on performance. This model recognizes efforts and those who make the most efforts get the greatest reward. Dr. Wheelis relies upon a large number of part time clinicians. This means more clinicians are employed as compared to if they were employed on a full time basis.
Considering the affiliate model Dr. Wheelis is thus revenue oriented and has to depend on the clinicians to get the revenue. Having this model working especially in a healthcare setting can be very difficult since lack of proper management and control is likely to cause inefficiency just like TBC’s case. TBC has to accommodate a large number of clinicians as they are working part time. If a good amount of revenue is not created, this would mean a loss. The clinicians have to deal with the patients at a very personal level leaving very minimal opportunity for the hospital management to attend to the patients’ needs. This model if of more patient to clinician relations and TBC just act as a facilitator of the two under the affiliate model. This resulted into the hospital getting into a state of crisis.
Dr. Wheelis should consider a model that employees a considerable number of clinicians and also TBC management should have a better command on the services the patients receive. A model which will accommodate a considerable number of clinicians and avoid any instances of overstaffing is preferred in this case. The staffing model with full time is the best model for Dr. Wheelis case with a healthcare setting that requires a large amount of quantitate data. The staffing model will ensure that there is no overstaffing and that the services are offered to the mental patients more responsibly. Efficiency will be restored at TBC with the right number of clinicians and encourage specialization leading to high quality service. This will considerably raise a good revenue for TBC.
