It has been brought to the notice of the Human Resources Department that you have been showing up late repeatedly. The electronic record shows that you have been late by fifteen minutes or more in February, (3, 8, 16 & 22). Although it is believed that your supervisor has taken up the issue with you on previous occasions, the office code of conduct dictates that a formal warning is issued to you. Therefore it is requested that arrive at the workplace at the stipulated time in future, failing which corrective action will be taken against you.
Note to the supervisor–
We believe that Ms. Conrad is a very efficient worker otherwise. Therefore we suggest that the warning be delivered to her tactfully so that is has no negative implications on her performance. We advise that you communicate to her that the organization appreciates her work and her compliance with punctuality will be helpful in recommending her for a promotion.

In response to the written request from your section supervisor recommending your immediate termination, the Organization has taken serious notice of the charges on you and has decided to terminate your services w. e. f the date of receiving the memo. It has been brought to our notice that you have been playing pranks in the workplace not long after your appointment on January 5th 2016, and in spite of repeated warnings from your supervisor you have not been deterred from indulging in your childish behavior. On the contrary your supervisor has observed that your pranks have escalated and is concerned that they may jeopardize the safety of the warehouse area. Your recent prank of making a maze of ladders in the warehouse has hurt an employee in the process of disassembling them. We are convinced that your activities may endanger others seriously in the future. Therefore we have no other options other than to terminate you with the warning to keep away from the organization premises.
