Parish nurse role was envisioned to integrate faith and health through enhancing personal health counselling and basic health education. The role also extends to acting as referral agents and advocating sustainable health in communities. According to Health People 2020, the parish nurses have intervened raising the various fundamental objectives in implementing primary health care. Some of the objectives include; anticipating for proper healthcare guidance, protecting and supporting those who have been abused, enhancing spiritual growth and facilitation. Other objectives consist of safe care advocacy, medication management, stress reduction amongst community members, spiritual support, enhancing hope and inspiration amongst community members, health education, health system guidance and environmental management. This paper will present a clear elaboration on how Faith Community Nursing have facilitated on the achievement of the interventions discussed herein and how they collaborate with parish nurses to ensure effective primary health care provision(Blais, 2012).
Faith communities have proposed for actionable models that vividly elaborate on nursing tools, methodologies of evaluation and assessment health care in the community in order to attain a sustainable primary health care. The model also indicate how feedbacks of intervention, dissemination and assessment will help in achieving Health People 2020 Goals. The community nurses emphasize on minorities’ health assessments, evidence –base evaluations practices through strategizing future Health Goals. The action model provided by Faith Community also outlines policies, programs, and information reliability to enhance best patient outcomes. These outcomes include behaviours, risk factors, injuries, diseases and other health related quality and equity (Hickman, 2006)
Faith community nurses network and administers roles while working with parish nurses with the aim of providing intentional healthcare .the nurses also educate on health related issues and support individuals and family members as well as parish Nurses. Such networked partnerships will facilitate good communication, eliminate health disparities and increased IT nursing practices (Hickman, 2006).
While providing basic primary health care, the state health departments play a role in administering health services and offering resources to public health centres facilitating continuous health care amongst the community members. The departments also provide medical aid to people with chronic and severe ailments. They also provide health planning, implement public healthcare management and assess public health care programs
In conclusion, collaboration facilitates better outcomes from patients and provides good plans of patient care. It also facilitates effective communication, and discipline while at work. Nurses can talk to individual patients to enable them understand their beliefs and avoid barriers. They can as well educate them to create and awareness.
