There is the need for seeing to it that the care which patients receive has been founded on evidence since such a move is aimed at achieving improved outcomes on the patient. In the same regard, the way a patient is assessed assists in developing the best forms of care which will adequately address all their healthcare needs. Therefore, the questions which I would ask my patients during all their visits for the purposes of providing them with individualized services will include the following.
How many times have you visited this healthcare facility?
Have you ever received some vaccination in this facility?
Have you ever been vaccinated in any other facility apart from this?
Was your health information documented during your previous visits?
The age and gender of an individual to a great extent affects the approach which an individual will employ in the delivery of health education and prevention services. For instance, there are particular immunizations which are given at birth to help in building the immunity of the child against possible future illnesses. Polio vaccine is used in preventing the child from getting paralysis while erythromycin is given to protect the infant from possible sexually transmitted infections effects to their eyes. As regards to gender, particular preventive measures may be gender specific. For instance, whereas breast cancer screening may be done to women only, prostate cancer screening is specified for the male population (Centers for Medicare and Medicaid Services, 2015).
A patient’s developmental milestone plays a significant role as far as their health and wellbeing is concerned and the initiative which an individual takes for ensuring their health. For children and adolescents, the primary responsibility for their care lays squarely on their parents (MedlinePlus Medical Encyclopedia, 2015). For individuals that have attained the majority age, the ability to make decisions regarding their health is totally dependent on themselves.
For any health promotion program, there are particular factors which may result in such a program not being achieved. Issues like overweight and obesity, insufficient physical activity, excessive alcohol consumption and imbalanced nutrition may compromise the expected benefits of a health promotion program (AIHW, 2017).
