- Failure to document, report, or record information accurately, promptly, and properly
A nursing assistant needs to be extremely meticulous and diligent in practice. Documentation of each clinical activity administered on patients must be recorded in the patient’s case file. Mostly, nursing assistant forgets or misplaces by accident the recording of important medicines administered. Also misreporting of smaller details may lead to fatal health conditions in a patient. (Davis, 2001).
- Failure to properly assist a patient while ambulating, getting out of bed, or during transport.
Sometimes nursing assistance tries to work on the delegation assigned. In the spur of the moment, they tend to think like the victim and act recklessly. It is very essential for the clinical assistance to carry their duties prescribed in their roles. Without panicking along with the patient and its supports, a nursing assistant needs to practice the treatment procedures taught to her during while shifting the patient like ambulating, moving from bed or during a transport.
- Failure to properly position a patient
An essential part of the nursing training is how patients are placed under different conditions of illness. While these positions seem very common and nothing special to remember, except few emergency cases, small negligence on the part of the nursing assistant for certain kinds of illnesses and injuries can cause serious damages to the patient (Davis, 2001). Proper positioning of a patient especially before and after operations and during certain tests is very vital for accurate diagnosis and cure.
- Unsafe placement or positioning of equipment of medical devices
Although most of the necessary medical equipments are machines which come with proper protection and covers, some of the equipments like the surgical instruments and other post-operative equipments need to be absolutely exactly placed according to the chart made. For e.g., if the bottle of blood is not positioned properly, the patient may not receive the blood and even die (Foster, & Faut-Callahan, 2011).
- Failure to properly monitor a patient under observation
Most of the serious patients are kept under the monitoring of the senior and more experienced nurses (Foster, & Faut-Callahan, 2011). However, some negligence like, sleeping at work, missing on a vital signal or even forgetting to monitor certain timely records may be punishable by law under the depending on the condition of the patient.
