Interview with an Older Adult:
Introduction
The interview was performed to evaluate the health as well as wellness assessment of Antony Crum one of the elderly man in our neighborhood who is currently seventy-five years old. The interview was performed at his home during the daytime. Various assessment tools were used to carry out the interview and the responses collected were used to plan a nursing intervention care for Antony Crum.
The following assessment tools has been employed to help in carrying out this interview and the result of each tool is indicate, although the form used have not been attached in this paper.
The past social life of Anton Crum was assessed by questioning the close family members such as his wife, elder sisters as well as brothers. His medical history was also considered by looking into his medical records. It was found that Antony had minor eye problem at his youth stage. Other social historical information that were looked into are religious as well as spiritual life, family dynamics, past occupation and also the status of retirement.
Nutrition Screening Initiative Checklist
This was performed by providing Antony with a designed form which he had to fill by circling a number corresponding to his eating habit to him to help in determining his nutrition health. Antony scores four in his nutrition taste which indicates that he was at moderate nutritional risk (Folstein et al., 1975).
Katz Index of Independence in Activities of Daily Living
This was used to assess Antony’s functional status in addition to his ability to carry out daily living chaos independently. He scored 3 out of 6 which is the maximum rating. This means that he is partially independent because there were some activities which he was unable to perform alone (Folstein et al., 1975).
Mini Mental Status Exam
This tool was used assess the mental status of Antony Crum by testing at most five functional cognitive areas, namely; language, ability to recall, registration, orientation as well as attention or else calculation (Olasunbo & Ayo, 2013). Out of thirty, Antony was able to score twenty-four which shows that his degree of impairment is mild.
Fall Prevention Scale
In this tool, a high score means the person is the fastest behavior while low scores indicate that the individual is the riskiest behavior. In answering FaS thirty statement, Antony’s result shows that he falls under the “often” category.
Summary of Overall Impression
The general health of Antony Crum was good because, from the assessment results above, his results were average. Although he needs health caring program because of his age, he still looks strong as well as energetic health wisely. Additionally, the body function of Antony is moderate, because he can eat good diet every day and his digestion system do function well (Schaefer, 2015). His respiratory system is also working fairly well since he does not suffer from any lung disease.
However, due to lack of fulltime care from an energetic person, since his wife is also old, Antony Crum might face some serious problem shortly. As he continues to grow older, performing physical excises will be difficult for him, and he must need a helper so as to be physically fit. Another problem which Antony may face very soon is the loss of his lower teeth, and this will force him to acquire artificial teeth in addition to order particular types of food daily (Schaefer, 2015). This is in relation to the historical information which shown that Antony’s father lost his teeth at the age of seventy.
Medication List
Antony new the name of most of the above medication and even how to use them in his condition wherever necessary. Although he was not well conversant with the herbal medication.
Conclusion
Performing health care assessment to older persons is very important because it will help to reduce their health risk as well as making it easier for them to access the right medical treatment in case they fall sick.
