Scott Klein, 38 year old male, coherent with normal appearance. Mr. Klein exhibited pressure of speech. His memory was normal. Client posture was tense. He reported that his sleeping patterns had been erratic. He indicated his appetite was stronger than usual and he could not eat enough to satisfy his hunger. His attitude toward interviewer was initially polite. Client facial expression was nervous and tense. His attention was vigilant.. He showed clear signs of euphoric mood. He described his mood as extremely happy but mentioned he was worried about home invasion. His affect was heightened and dramatic. Client denied and showed no indicators for depression. He displayed indicators of hypermania, including rapid speech, hyperactivity and profuse sweating. He exhibited signs of intermittent anxiety. Client reported that he was seeing people in the bushes outside his home and believed they were monitoring his movements. His thinking was disorganized and he was unable to focus on questions. His thought process was erratic and unfocused with flight of ideas. He denied suicidal and homicidal ideation. Clients thought content was obsessive. Client showed no indicators of self-harm but threatened to protect his home from surveillance, indicating secondary delusions, a paranoid ideation and strong potential of harm to others. Client denied hallucinations. He showed signs of psychomotor agitation, including dyskinesias. When asked if he was irritable, Mr. Klein responded that he believed he was being followed. Mr. Klein displayed He initially deflected question on substance abuse but later specified he was taking OTC sleep-aid medication to calm anxiety. His mood appeared to cycle rapidly and began to show signs of hostility.
