Bennie Bellamy was employed as a Certified Nurse Aide (CAN) at PFCH in May 2002. Bennie Bellamy’s discipline improved with time. A short while after his employment he rated lowly on dependability after failing to observe some medical procedures on a patient. He was issued with a corrective action as a result. Bellamy scored an overall average of 1.9 in his evaluation after his first year at PFCH. In his second and third years at PFCH (2004 and 2005), Bellamy improved in his discipline and obtained an annual overall score of 2 (Acceptable). Bellamy, therefore, exhibited progressive discipline while working at PFCH.
Ms. Branham Alva was employed at PMCH as a security officer starting September 1995. Overall, she exhibited outstanding progressive discipline. Though she occasionally made some mistakes her response and discipline were impressive. In the first year (1996) she scored an annual average of 2 (Acceptable). She did better in her second year by scoring 2.20. In the third year, Branham’s score declined to 1.85. She had been ill for sometime causing her to miss work. In the subsequent years, Ms. Branham impressed with her discipline scoring 2 on three occasions, 2.125 in her fourth year and 2.06 in her ninth year at PFCH. Only twice did she register a score below 2. During her time at the hospital Ms. Branham comes across as an employee whose discipline was satisfactory.
