Part 1
Compromised levels of safety in her house
During the nurses’ visit to Ms. Fisher’s house, a couple of factors were observed to expose her to dangers that were likely to impact on her wellbeing. The level of lighting in the house is low; therefore, her chances of tripping and falling over are very high which may result in her breaking bones. Secondly, she is wearing slippery shoes. This aspect added to the realization that the rug in the house is not placed appropriately may result in her falling and hurting herself.
In this direction, there is the need for nursing interventions to be developed with the intentions of improving Ms. Fisher’s wellbeing. As such, the nurse is supposed to help her arrange her house properly to prevent her from knocking items over and falling which might result in her breaking her bones. This comprehension is also important as it will lead to proper lighting in her house. Additionally, she also needs to use corrective lenses which will help improve her eyesight. Finally, she should take physical exercises which are vital for strength and reducing and her frail nature (Kisner & Colby, 2012).
Not complying with her medical prescriptions
The video gives a clear depiction that Ms. Fisher’s pills have not been well arranged. As such she is under the risk of using the wrong dosage of her medication. Also, she has got problems regarding her adherence to the medication that she has been prescribed to saying that she does not need all that medicine. Finally, she informs the charge nurse that she does not require any oxygen supplement even though it was recommended by her physician.
The interventions for this case are supposed to dwell on the apprehension that for her wellbeing to be boosted there is the need for her to stick to her medical prescription. To oversee this fact, it is imperative that a care provider visits her to assess her adherence to her medication (Scott & Winters, 2015). Additionally, since she is battling with chronic heart failure, it is important that she sticks to her oxygen supply to avoid her condition worsening through exacerbation.
Dietary problems
Ms. Fisher informs the charge nurse that she has lost her appetite over the past few days in addition to not having any bowel movements. Such a realization has led to her losing more than 6 kilos during her hospitalization period (Grand Canyon University, 2016). She links her loss of appetite to her husband’s demise and the resulting despair. Additionally, her skin shows signs of lost turgidity.
Situations of despair
The video also shows that she has got signs of depression. The fact that she mentions her deceased husband implies that she is missing him which increases her loneliness. This realization is evidenced by her weight loss, sadness, defiance, and social withdrawal. By her having a lot of newspapers in her house, it implies that she struggles with social withdrawal since she is a loner.
Part 2 Dialogue
Nurse: Good morning Sallie Mae. My name is Ms. JL, and I will be your nurse today. I understand you are experiencing problems with your wellbeing. According to your patient information, you are struggling with high blood pressure, congestive heart failure, and dyspnea. Were you given the required education concerning how well to take care of yourself?
Ms. Fisher: Well, my physician took the initiative of informing me the steps that I have to take in maintaining my well-being including the medication and diet. I am however not in a position to stick to this program because I am lonely.
Nurse: It is okay Ms. Fisher. I would like to inform you that it is imperative that you observe your medication regimen to the latter. Sticking to your prescription plan is an important factor towards addressing dyspnea, congestive heart failure, and hypertension.
Ms. Fisher: I am getting your point madam.
Nurse: Additionally, judging from your condition, it is also important that you take physical exercises which are an important factor towards addressing CHF. Exercises also boost your levels of self-esteem which are essential for addressing your depressive status. Taking care of your nutritional needs regarding incorporating fruits and vegetables into your daily diet is a vital factor in improving your blood levels and your eyesight. Moreover, it is of great importance that you arrange your house in a proper manner since it is a significant step towards the prevention of falls. All these efforts will result in your overall well-being.
Ms. Fisher: I appreciate all the information that you have given me and I am going to stick to it.
Nurse: It has been my pleasure interacting with you. Have a good day.
