Guided Study: DNP Scholarly Project Proposal Development
1. Setting
The DNP project will be conducted at JDH Medical Group which is a health care setting with a primary level of care. The typical clients in this institution are adults and elderly adults, majority of which are Hispanics. As a health care setting, the JDH Medical group aims to promote excellence in education, research, and patient care. Additionally, JDH Medical group has the mission to extend quality, accessible, and affordable health care services to everyone especially to the people of Miami Dade (e.g. prevention programs, comprehensive care, and highly complex care) and to work hand in hand with different institutions to establish a good learning atmosphere to everyone in the community. On the other hand, the vision of the setting is to accomplish all of the health needs of the Miami Dade community as well as its surrounding communities through the provision of high quality, accessible, and affordable health care services through its integrated delivery system (JDH Medical Group, n.d.).
The organization is consisted of different missions related to health care, education, and even biomedical research. Through the approval of the Institutional configuration, the JDH Medical Group can become a well-developed academic medical center consisting of experts with experience in different therapeutic areas. With the help of the strong connections of the JDH Medical Group with investigators from other countries, it has emerged as a center for national and even international exchange in medical bioscience research. For the organizational structure, the JDH Medical Group consists of 1 Medical Director, 1 Medical Research Director, 7 Physicians, 4 Principal Investigators, 5 Sub-Investigators, 5 Clinical Research Coordinators/ Medical Assistants, 2 Regulatory Specialists, 1 IT, 3 Quality Assurance Specialists, and 2 Laboratory Technicians (JDH Medical Group, n.d.).
2. Organizational Need
Determining whether a particular implementation is an organizational need depends on the brainstorming and assessment of the different needs of the organization. For the JDH Medical Group, an organizational needs assessment and meetings with the stakeholders were done to point out the different needs of the organization. Upon conducting assessments and interviews, it was revealed that the center does not have any protocol or guideline for the implementation of vaccinations among the adults and elderly adults population. Since the elderly population are considered to be a high-risk population, it is necessary to develop guidelines and protocols for the vaccination of the said group. As mentioned in the Iowa Model of EBP, the trigger to initiate change can either be problem-focused or knowledge-focused (Dontje, 2007; Brown, 2014). For this issue, the trigger is considered to be a problem-focused trigger since the absence of guidelines and protocols for vaccination is considered to be a risk management issue.
3. Stakeholders
The main stakeholders for the DNP project are professionals who are directly involved in the brainstorming and implementation of the guidelines and protocols in the center. In the JDH Medical group, the stakeholders include the family nurse practitioners (FNP), registered nurses (RN), MA, and medical doctors (MD).
4. Organizational Support
Since the identification of the absence of guidelines or protocol for vaccination of the elderly as an issue was determined through interviews and brainstorming of the stakeholders of the project, it is apparent that the DNP project is supported by the organization. All of the necessary support needed for the DNP project was obtained including financial support.
5. Sustainability
It is expected that the practice change can be sustained upon the completion of the DNP scholarly project because the development of protocol or guidelines for the vaccination of the elderly population can be easily sustained after the implementation. The said project can be sustained through collaboration of the different stakeholders to continue the implementation and update of the vaccination records and schedules of the elderly patients.

The project is a minimum risk project because it only include addition of protocol and guidelines for the vaccination of the elderly Hispanics.
