Patient Management Delivery Care Systems:
In US there are three types of Patient Delivery care systems and I have been exposed to in Nursing School. They are: Private, Public and Government.
Private Management Delivery Care System:
Private Health care system is the one that is premium insurance based, in this system profits are important to pay for staff and to buy equipments. Private health care consists of few organizations, which are PPO (preferred provider organization), preferred provider care and HMO (health maintenance organization). All the three organizations differ in their delivery design but are same in their participation to health delivery.
Public Management Delivery Care System:
Public or socialized is the health care system where Hospitals, Doctor, Nurses, Machine, drugs etc are paid for by government via taxes from employed. Here government supply provisions under Social Security Act. At Federal level medicare is provided and at State level Mediaid is. After the age of 65, Medicare facilities are provided to all eligible citizens. The services are provided with government established fee structure for Physicians. 80% of fees is covered under Medicare and rest 20% is covered under insurance. There are yearly reviews of budgets and benefits of Medicare services. Physician who participates has to provider services based on such criteria and accept the allowable fee. Government Management Delivery Care System:
Under this system Health care of uniformed personnel, their dependent and retired military personnel are taken care of. The program is called CHAMPUS (Civilian Health and Medical Program for Uniformed Services).
Nurse Patient Ratio:
In California, primary care nursing is where the patient care done by an RN. This is done under the ratio of 1:5. Team Nursing is the delivery where a patient is taken care by a team of nurses, where RN controls proper functioning of all the team members. The system has been very successful in delivering a proper caring system to the patients and hence must be replicated by other states and eventually other countries also. This way this services of Nursing where human life is of utmost importance can be taken care of. (“Department of Health”, 2)
