I agree with your discovery that even though continuum of care is defined differently in different settings, there is a confluence of goals and objectives. I also agree with your point that the lack of integration and cooperation among service providers is a big impediment to the continuum of care. This also relates to the differences in business objectives and care objectives, and the fact that business objectives might gain precedence over care objectives (Finkelman, 2011). The use of social workers and case managers are ways that my organization has used to achieve continuum of care. However, utilization is new to me, and your discussion forum did not explain this concept further for my knowledge.

Hello Gina, your approach towards continuum of care is refreshingly simple and easy to comprehend. In your opinion, the role that case managers play towards achieving the continuum of care is indispensable. While I agree your perspectives here, I also laud your ideas on the transition from one level of care to another and the importance of the provision of health care. This vindicates your arguments that case managers are very important to the achievement of continuum of care (Mullahy, 2013). I believe you are an asset to your organization. Your knowledge on the importance of case management will play a role on the establishment of this unit.
