Gordon Functional Health Pattern assessment involves assessing 11 aspects of the person’s life. The first is to gain a perception about the general concept of health and well-being to the person. Asking question regarding the general health of the person’s family and what they do to remain in good health would give a lead on this. One can also asses the individual for unhealthy habits like smoking, tobacco use, unsafe sex, etc. The degree of compliance with treatment or self-management of disease can also provide an idea about the individual’s general perception about health. The other criteria of assessment, ii. nutritional status, iii. status of eliminating bowels and urine, iv. level of physical activity, v. sleep and relaxing status, vi. perception and cognitive ability, vii. perception about self, viii. relationship with family and social group, ix. sexuality and reproductive health, x. ability to cope with stress, xi. spiritual beliefs. I have not participated in any health screening intervention so far, but I do think about participating in a mental health screening intervention. I am trying hard to maintain positive mental and would like to access my mental status from time to time and take necessary action. I rarely visit a doctor, and have never given a thought of visiting the clinics for a regular assessment. I usually mange my health problems with herbs and over the counter drugs. As per Gordons functional health pattern assessment, I score positive on perception about health. I do not entertain habits that can cost me health. I neither drink alcohol nor smoke tobacco. With respect to nutrition, I have regular bowel disturbances. I am not sure why I have it. I have never thought of following upon it seriously. I manage it with ginger and sometimes with antacid. I have a habit of skipping meals and don’t feel hungry often.
